Student Work Permits

Working Papers

If you are under 18 years of age, you must obtain a work permit before starting a new job per M.G.L. c. 149, 86-89. The following are the steps you should take; please note that the Physician's Certificate of Health is required only for 14 and 15 year-olds.

  1. Print the Employment Permit Application and ask the employer to complete and sign the Promise of Employment.
  2. The student and a parent/guardian must then sign the application. A physician must sign the application if the student is under 16 years old.
  3. Once signed by the employer, physician (if under 16), parent/guardian, and student, please bring the papers and a birth certificate or valid U.S. passport to the Main Office at Chapman Middle School (1051 Commercial Street) or the Administration Building (89 Middle Street). The student must be present to sign the permit.
