Student Work Permits

If you are under 18 years of age, you must obtain a work permit before starting a new job per M.G.L. c. 149, 86-89. The following are the steps you should take; please note that the Physician's Certificate of Health is required only for 14 and 15 year-olds. 

  1. Print the Employment Permit Application and ask the employer to complete and sign the Promise of Employment.
  2. The student and a parent/guardian must then sign the application. A physician must sign the application if the student is under 16 years old.
  3. Once signed by the employer, physician (if under 16), parent/guardian, and student, please bring the papers and a birth certificate or valid U.S. passport to the secretary in either the Maroon or Gold offices at Weymouth High School. The student must be present to sign the permit.
