Required Documents for Registration

 

The following documents are REQUIRED for enrollment at Weymouth Public Schools.

 

Please make sure you have all your documents printed and prepared for review. If you have documents on your phone please download and print them before you come to your registration appointment.

 

No student will be enrolled without completed registration documents.

☐ Your child’s birth certificate

Student birth certificate must have a raised seal.

☐ Your child’s most recent physical exam and record of immunizations

If your child will need to take medication during the day, please also provide the doctor’s orders.

☐ Your valid photo ID

Massachusetts ID, driver’s license, passport, or other government-issued photo identification.

☐ Proof of residency

Homeowners must provide 2 of these documents: Copy of deed Most recent mortgage payment Property tax bill Most recent property tax payment Copy of settlement statement	Everyone must provide 1 of these documents (must be dated within the past 60 days): Cable/Internet Bill Electric bill Gas bill Water bill Oil bill W-2 form Payroll Stub Bank Statement Excise tax Renters Insurance
Renters must provide both of these documents: Copy of up-to-date lease signed and dated by both landlord and tenant Signed and notarized Landlord Living Agreement

☐ Student’s prior school records: most recent report card/transcript
 

☐ Other important documents, as needed

Active IEP, 504 plan, custody/guardianship agreements, court order, WIDA/Access testing scores
